Answer:You don't need to the doctor for one for this length of time, but you will need to get it from the receptionists at the surgery.
Think you'll find you need to go to a doctor to get a sick note, not a chemist
A self cert sick form is usually one that your work will provide you with. Its just basically a declaration that you have been sick and not sciving!
Other wise you get a sick note from your doctor.
Chemists/pharmacys have nothing to do with sick notes.
