My best suggestion is to seek a psychiatrist or a physician that specializes in neurology.
Consumption of caffeine, nicotine, chocolates although do not in themselves cause panic attacks, they will dramatically AMPLIFY the severity of them. If you want to regain control then stop using these things... especially in excess. Any level of use of nicotine is excessive with regards to panic attacks. Quite often you will notice a surge in anxiety , rapid heart beat, tremors about 1 +1/2 - 3 hours after consuming coffee, tea, chocolate bars, pepsi, mountain dew. The amplification can be enough to make you want to call 911 or get checked out in the E.R. If you want the severity to improve, drop the use of these items.
I understand completely what's going on. I suffer from OCD, which is an anxiety disorder. I took Zoloft for five months, and it did not seem to work for me so I got off of it. But don't take Xanax if it's not perscribed by your doctor, even if it's helping. Tell your doctor and he/she will help you out. There's other SRI drugs that you can take, but ask your doctor. Wow your problem sounds very familiar. I too have severe anxiety disorder. I have been struggling with it for over 16 years now. I have been to countless doctors and I've been prescribed every medicine you can imagine. I was on Zoloft for over 5 years and it never helped me at all. Also, I have no insurance and Zoloft is VERY expensive. The doctor I was seeing at the time just would not give me anything else. He wanted to keep upping my dosage of the Zoloft instead. He said we would eventually find the proper dosage I needed to work for me, but it never happened. I felt just like you described - like it was making my anxiety attacks worse instead of better.
