It depends really. For some it is due to the force in which the urine hits its destination (toilet, floor) thus causing it to mix with air causing bubbles. But foamy unrine is usually due to proteinuria or albuminuria (presence of proteins in the urine.
Normally the kidney is supposed to filter your urine (through filters called glomerulus) thus retaining the useful substances (glucose, protein, etc) and excreting the toxins (uric acid, chemicals from some drugs etc). However when there is a disfunction of the kidney eg the filtering mechanism not working properly, some substances may escape into the urine eg protein. But that is not to say that you have a kidney disease, it could be caused by a high intake of protein and even in pregnancy.
