I believe you should have received a letter from his secretary, letting you know of his death and therefore giving you the option to either con't on being a patient under who ever took over his practice or sent you a record release form to authorize the release of your medical records. If they haven't maybe you should write to the office requesting the release of your medical records, and see what happends. Good Luck.

You need to try the office first, the office manager or covering doctor would have the authority to release the records. If the doctor practiced alone then you will have to contact the local medical board to see who has the files. Unless the daughter is a doctor then she should not have access to the patients medical records that is a HIPPA violation.

My doctor died years ago and I recently found out that his practice went out of business.I had to go to my local hospital and signs forms to have my records sent to a new doctors office.You may want to try your local hospital, they should be able to offer you some information.Good luck
