40 years old, smokes black and mild !
Answer:Get um checked for diabetes.
First thing comes to my mind is water retention caused by something wrong in the heart that needs investigating fast. 40 years old and a smoker makes him a prime candidate for heart attack. Get him to a doctor.
In this particular case, smoking orobably has nothing to do with it.
Swelling of the ankles, feet or fingers could mean high blood pressure, or even heart disease. This can also be caused by drinking alcohol (the liver). If he has gained weight recently it could be caused by that as well.
Don't let him add salt to his food (remember that mayo, mustard, ketchup, A1, gravies, canned veggies and other canned foods, all contain salt--as does chocolate--cake and cookies). Hot dogs and most lunch meats are full of sodium--don't let him eat them.
Look for "Low Sodium" products when grocery shopping.
He should see a doctor immediately--this could be serious.
I would say he is retaining fluid, and that is usually a sign of high blood pressure. He should go to the doctor, have his blood pressure checked, QUIT smoking & he may need a diuretic. But check with the doctor ASAP !
How is his blood pressure? He may be retaining water as well. cut way back on salts and drink a lot of water to flush the system, avoiding caffeinated beverages as well. Have him elevate his feet as much as possible, and also walk around helping with circulation. Smoking does not cause this, though it does not help, if he has any cardiovascular problems. If this does not clear up and you do not still know the cause, it could be a dangerous situation, as this is a possible sign of congestive heart failure. If breathing becomes difficult call 911

He's in early congestive heart failure; he's probably got a blocked cardiac artery from cholesterol compounded by constriction of the artery due to nicotine. He needs to go to ER; if he's as stubborn as most men that age, make him sit with his shoes off, feet raised up preferably above the heart, restrict his fluid intake esp alcohol and caffeine, no salt...If he gets short of breath, ignore his desires and call 911 if he won't be driven to ER. He needs to be seen very soon.
