Click on Locate a Provider and then Dentists and enter your zip code. It will bring up a list in your area and their numbers. You can call them and ask them how much a tooth implant would cost and then ask them what it would cost if you used the AmeriPlan benefits.
You would only be paying $11.95 a month for the coverage itself so it is a life saver!!

In Chicago we charge 1850 for an implant. That includes the procedure, implant and healing abutment. I doesn't include the impression, crown, permanent abutment and DDS services for crown work.
